Support for Veterans, Designed for Real Life

We deliver occupational therapy services under the DVA’s Rehabilitation Appliances Program (RAP) and Veterans’ Home Care (VHC) to help you stay independent, safe, and mobile.

All DVA services must be initiated by your GP using a D904 referral form.
We are unable to begin any assessment or service without this completed form from your doctor.
